Guidelines for Submitting Reserve Request Forms

If you want your course materials to be accessed in libraries only, fill out the Request Form for Hardcopy Reserve. To place materials on the Web or add a link to your course-related homepage, you may fill out the Request Form for Electronic Reserve. Instructors are required to :

1. Fill in the course information at the top of the form. You may enter "none" if course code or course number is not applicable. Enter your e-mail address if you want to receive a copy of the request form that you have sent. Except for e-mail field, the form will not be accepted with any blank course information fields.

2. For Hardcopy Reserve, you may complete the information about the reserve materials in English or Chinese : author, title, call number, loan rule, etc. Then click on Submit button. If you want to request multiple items, click on Submit and continue to add items button.

3. For Electronic Reserve, you will be given a reference number (e.g. 924250869) after submitting your request form.

   You can submit the non-copyrighted course materials :

• as an attachment by email to a server at resbk@lib.cuhk.edu.hk, typing
  (a) Electronic Reserve at the Subject Line
  (b) Reference Number (e.g. Ref no: 924250869) in the body of the message

• as an electronic file on disk, including the reference number on the cover. Send the diskette to the Reserve Books Department, Univeristy Library.

• as as a paper copy, including the reference number on the title page. Send papers to the Reserve Books Department, University Library.
